CPGHA Annual General Meeting Forms

Below you will find forms pertaining to the Annual General Meeting (AGM).

A Proxy Form is used in the event that you cannot physically attend the AGM and would like to have your vote counted. For the purpose of ensuring that all proxy voting is subsequently verifiable, the Executive requests this Proxy Form be used at the AGM.

The Self-Nomination Form is used by CPGHA members to indicate their intention to be a candidate for election to the Executive.

The Nomination Form is used by a CPGHA member who would like to nominate a fellow CPGHA member to be a candidate for election to the Executive.
