** PLEASE NOTE**  Failure to provide this document will result in being asked to step down from your Coaching or Staff position.
ALL Coaches, Bench Staff and Executive require a MANDATORY Police Record Check every 2 years.  Fill out the form and bring it to the OPP Station.  The process usually takes around 1 week. 

For Bench Staff and Volunteers
